This service offers you a straightforward and thoughtful way of leaving your requests behind, saving your loved ones the upset of making further decisions during a difficult time, without a financial commitment.

The form below is all you need to confirm your funeral wishes. Once you have completed the form you will receive an email of your wishes to keep with your other personal documents. Should the unexpected happen, your wishes are clear and ready for your loved ones to follow.
